DIRECTOR OF CASE MANAGEMENT

Appropriate utilization of resources promote and facilitate patient movement through the healthcare continuum through development and implementation of processes and procedures which will ensure timely and proactive Case Manager and Social Worker interventions. Coordinates, supervises and evaluates the Case Management program in order to facilitate cost effective, safe clinically appropriate and quality care.

Required Education: Bachelor's degree in Nursing

Required Experience: Case Management managerial experience

Required Licensure/Certification: CA RN License or Master's Degree Preferred
